FOR U.S. BORN & U.S. PASSPORT HOLDERS, please send or bring in the following 5

1. ORIGINAL PASSPORT: It
has
to
be valid for at least 6 months for single or double entries
application; one year for one year multiple entries application AND at
least one blank visa page.
2. VISA APPLICATION FORM: Download,
complete
online,

3. PHOTO: One
passport
photo
(2" x 2" with white or blue background).
4. COPY OF TICKET OR ITINERARY: Proof
of
traveling
dates from U.S. to China.
