2.      Passport holder was over 16 when passport was issued; and

3.      Name has NOT been changed in current passport; and

4.      Passport is not damaged

                    No homemade digital photo accepted)

STEP 3         MOST RECENTLY ISSUED PASSPORT (Valid passport OR Passport that has expired less than five years

                    and was issued when holder was over 16 of age)

STEP 4         COMPLETE AND SIGN AUTHORIZATION LETTER (click here for Authorization letter)

STEP 5         COPY OF ITINERARY OR AIR TICKET (proof of departure date from USA)

STEP 6         PAYMENT TO ABLE TOURS (click here for instruction & credit card authorization form)
